You can have as many punch buttons as you want, shoulders or not. The only thing you can't have is a [P]+[K]+[G] button, [P]+[G] button... Go on...
Yes you can either choose pad or stick. You can also customize your button layout but single p+k, p+g... buttons are not allowed.
